The links below will bring up pictures of the Construction Permit application forms.  They are here to give you an idea of the information you should have with you when you come to the Construction Department to apply.  You may print these forms and gather as much information as you can so you are prepared to fill out the real forms at the Construction Office.  The real forms are "4 copies - press hard", so you can't use this image.

Some of the entries on these forms are not intelligible to the applicant.  Don't worry.  A member of the Construction Code Staff will help you when you come to apply.

The person doing the work must apply, because the person doing the work must sign the application.  If you hire a contractor, he will apply.  If you plan to do the work yourself, you should apply.  AS an OWNER/OCCUPANT (only), you DO NOT HAVE TO HIRE a licensed electrician or plumber to do the work, if you are qualified to do it yourself (i.e. well enough to pass inspection.)
